At ExcellentPhysio, data has been collected on the registration form and contact forms via telephone, email or other form of communication.

At ExcellentPhysio, Patients will be asked to provide correct information as follows:

2. Hobbies and social status
3. Employment type
4. Records of phone, email or verbal communication
5. Payment details
6. Records of attendance
7. Details of GP and other health professional involved in patient's care
8. Details of current problem
9. Detailed medical history and previous diagnosis

Patients are advised to inform ExcellentPhysio of any change in the information given.

Use of personal information
At ExcellentPhysio, with patient's permission, patient's contact information may be used to send out occasional promotional offers or more information about services and products which might be valuable to patients. Patient's information may be used for conducting market research. Patient's information can be handed over to Police, Solicitors or Immigration Officers if legally required by Law.

Text Messaging
At ExcellentPhysio, patients will be contacted through SMS to notify or remind about appointment.

Emails
Patients may be contacted from time to time regarding changes in services or promotional offers about the services through email.

ExcellentPhysio follows strict security procedures in the storage and disclosure of information which have been provide by patients. All patient identifiable information is kept secure. Only therapists who work at ExcellentPhysio have access to such information. This is to prevent unauthorised access or unlawful processing of any personal information.

Retaining Patient's personal data
At ExcellentPhysio, all personal data will be kept for as long as necessary to fulfil the purpose of collection.

Non-disclosure to third parties
At ExcellentPhysio, patient's personal data will not be disclosed to third parties unless patient has given their consent. ExcellentPhysio will never sell, exchange or trade patient's personal information to others for marketing purposes.

Patient must mention to the therapist at the initial assessment if they wish to opt out.
